How to register
In order to register with the practice, you should bring your NHS Medical card to the surgery. If you do not have a medical card a form can be provided. If you are on medication, you will be offered an appointment with a doctor to discuss. You will be requested to complete two forms. The 'New Patient Questionnaire' form can be downloaded here for you to complete before your visit to the surgery. The GMS1 form is available at reception. Once these two forms have been completed you will be registered with the practice.
Appointments
We run all our surgeries by an appointment only system. Appointments can be booked by telephoning the surgery on the day you wish to be seen. A few appointments are available to be booked up to a fortnight ahead. All appointments are 10 minutes long, so if you have complex problems please ask to book a double appointment.
We try to make sure you see the doctor of your choice but if they are not available we may ask you to see another doctor.
If you are too ill to come to the surgery and you require a visit at home, please call the surgery before 10:30am. Please state whether you require a visit urgently (to help us assess priority). Home visits take up much more of a doctor’s time than a consultation in the surgery, so if you are mobile please come into the surgery to be seen.
Automated check - in
When you arrive at the surgery for your appointment, look for our 'automated check-in' which is located on the wall near to the town entrance. With just five simple steps you can check yourself in for your appointment without having to queue at reception. This is ideal for those patients who just wish to let us know that they are here. You will still need to see the receptionist if you have any other requirements.

- Touch the screen to select whether you are Male or Female
- Touch the screen to select which month you were born
- Touch the screen to select which date you were born
- Select 'yes' if your details appear on the screen correctly.
Watch the screen for confirmation that you have been checked in and then take a seat until you are called.
Booking an appointment online.
The only appointments available online are standard 10 minute GP consultations. If you are trying to book any of the following, you will need to telephone us on 01787 475944.
- Smear test
- Blood test
- Annual review appointment
- Any nurse appointment
- A double appointment
- Diabetes, Asthma or Hypertension consultation
Before you can use the online service, you must visit the surgery and register with us for your unique password that will be required for booking online.
One of the following three forms of identification will be required when registering:
- Passport
- Driving Licence
- Council tax bill
If you registered with us for booking online with our previous computer system, please come into the surgery to register again as the passwords have expired.

| Vaccinations |
Routine and travel vaccinations for all ages are available during surgery times by appointment with our nurses. |
| Asthma |
Asthma is very common now, and our nurse has been trained to run a clinic-type service. Modern asthma care has been shown to be most successful at both reducing symptoms and preventing attacks and exacerbations. Even if you have had asthma for many years, you should attend the nurse's clinic to make sure you are having the most appropriate regime for you; many asthmatics who have suffered for years have had their lives greatly improved by a more modern regime. Thursday 09:30 - 12:00 & 1:30 - 5:00pm. |
| Diabetic |
Our highly motivated Diabetes Specialist Nurse helps patients both newly diagnosed and already living with diabetes, to understand and manage their diabetes with a 21st Century approach. She offers six monthly and annual reviews. Tuesday 8:30am - 12:30pm and 1:30pm - 6:00pm. Wednesday 8:30am - 12:30pm. Thursday 8:30am - 12:30pm. |
| Heart Disease |
We have a trained nurse who will review all patients with coronary heart disease once a year. She will advise on lifestyle and ensure that you are taking the correct medication. Thursday 2:00 - 5:30pm. |
| High Blood Pressure | One of our nurses will carry out a regular review of people with high blood pressure. |
| Warfarin | Patients on Warfarin can have their regular blood tests carried out at the surgery and the results will be made available to them immediately. |
Repeat Prescriptions
If you are taking prescribed treatment this will be issued through our computerised repeat prescription service. There are six ways of obtaining repeat prescriptions: 48 hours (weekdays) is required, so please plan ahead - especially at holiday times.
- Between Monday and Friday, please telephone 01787 479889 with your request between 10:30am and 12:30pm.
- Place your repeat prescription request in the box in the waiting room.
- Send in your repeat prescription by post.
- Send in your repeat prescription by fax on 01787 474506.
- The local chemists operate a system whereby you can order your regular medication from them. They will obtain the prescription from us and have your medication prepared and ready for you to collect.
- Complete our on-line form which will e-mail your request to us. Your request will be confirmed by return e-mail.
We are keen to ensure that patients with ongoing medical problems are monitored regularly. If the date for your next review with a doctor or nurse has passed you will be asked to make an appointment.
Sick certificates
You only need a doctor’s note if you are unable to work and are ill for longer than seven calendar days. Your employer will provide you with an SC2 self-certificate form for shorter periods of illness.
Test results
Since our telephones are very busy in the early morning, we ask you to telephone for test results (eg blood, urine, x-ray) after 10:30am each day.
To ensure confidentiality, we only release results to the patient, unless alternative arrangements have been agreed in writing.
Non-NHS services
The NHS does not pay for some of the services we provide. These include private sick notes, insurance forms, holiday cancellation forms, medical reports, fitness to travel certificates, private prescriptions, passport signing and some vaccination services. Our fees for these services, which are in line with BMA approved national guidelines, are displayed in the surgery.
